Instructions for completing the
Universal Application for Admission/Attendance

The "Universal Application for Admission/Attendance" has been designed to be easily filled in on your PC using free software. There are several ways to complete the application.

Recommended
1. Open the file application1011.ott (an Open Document format template file) or application1011.odt (an Open Document format Document file) (Note: You may have to right-click on the file link and select "save link as" to download the file.) with the free OpenOffice word processor. Once you have made your selection in the first item you may tab from field to field selecting choices or filling in the requested information. When done you may save the file as an Open Document text document (odt) file using the student's first and last name, i.e. firstlast.odt. The file may be printed or submitted as an email attachment. If you have more than one student, you may use the first student's file as a template for subsequent applications.

OpenOffice, if not already installed, may be downloaded for all computers from http://openoffice.org/

Alternative 1
2. You may open the PDF version of the form, application1011.pdf, (Note: You may have to right-click on the file link and select "save link as" to download the file.) with Adobe Acrobat or the free Adobe Acrobat Reader. Fill in the form as described above. Users of the free Adobe Acrobat Reader, however, may not save the form with the data, so you must print the document on paper for submission. Users of the full Adobe Acrobat program can save the pdf form with data intact. Macintosh OSX users can print to a PDF file by default to save their completed form. Users with other PDF printer software, such as Linux cups-pdf or Windows CutePDF will also be able to print their completed forms to a PDF file. Such forms are suitable for submission as email attachments.

3. The blank form may also be printed and filled out by hand. However, this often results in our being unable to accurately read important information.
