Step 1: Athletic-Only Student Application (If Needed)

If your student is participating in athletics at Dominion Academy but is not enrolled academically, they must complete the Athletic-Only Student Application for eligibility. There is a $25 fee per student for admin purposes. Attendance at a local church is required for participation. Our head of school will review and approve every application. Once approved, you will receive your pay-to-play invoice.

Fill out the application here

Step 2: Athletic Registration Form

This form allows us to identify students who are interested in participating in athletics, collect student and parent contact information, and ensure coaches can communicate important team updates, schedules, and announcements throughout the season.

Fill out the Registration form here

Step 3: Physical Screening & Required Paperwork

All athletes must have current athletic paperwork completed and submitted before participating. Please submit completed forms to the school office or email them to athleticdirector@dominionacademy.org. Physicals are mandatory for participation and must be turned in before athletes can attend practice.

Required Documents

We are proud to partner with Kettering Health for all of our athletic training and sports medicine needs. To help athletes prepare for the upcoming season, Kettering Health is offering sports physicals on June 22 and July 16 for just $20. If your student needs a physical for athletic participation, this is a convenient and affordable option.

Step 4: Pay your Athletic Fees

After your application and paperwork have been submitted, you will receive an invoice for each sport through FACTS at the beginning of the current season. Uniforms are invoiced separately from pay to play. Payment must be completed before your athlete may participate — unpaid accounts will result in the player being withheld from play.

2026-2027 Fees

  • Cross Country — $300
  • Soccer — $300
  • Girls Volleyball — $300
  • Cheer — $300
  • High School Boys Basketball — $375
  • High School Girls Basketball — $375
  • Junior High Basketball — $300
  • Track & Field — $300
  • Boys Volleyball — $300

Step 5: Uniform Sizing

We are currently only collecting sizing for fall sports, winter and spring to come soon!

Head here to select your uniform size and number!

Step 6: Senior Jacket Order Form (Graduating Class of 2027 ONLY)

Senior athletes are eligible to receive a personalized varsity jacket. One group order will be placed before the beginning of the year so seniors may enjoy wearing their jackets all year long! All seniors- in any fall, winter or spring sports- should fill out this form if they would like to receive a jacket. The cost of the jackets is covered by athletic registration fees.

Jacket orders must be placed by June 30, 2026.

Step 6: Spirit Wear/Team Apparel Ordering

Uniforms will be distributed by the school but all other team apparel needs to be ordered individually. None of these are mandatory and can be ordered by any athlete, family or fan. Feel free to share the link with all!
