Step 1: Athletic-Only Student Application (If Needed)
If your student is participating in athletics at Dominion Academy but is not enrolled academically, they must complete the Athletic-Only Student Application for eligibility. There is a $25 fee per student for admin purposes. Attendance at a local church is required for participation. Our head of school will review and approve every application. Once approved, you will receive your pay-to-play invoice.
Step 2: Athletic Registration Form
This form allows us to identify students who are interested in participating in athletics, collect student and parent contact information, and ensure coaches can communicate important team updates, schedules, and announcements throughout the season.

Step 3: Physical Screening & Required Paperwork
All athletes must have current athletic paperwork completed and submitted before participating. Please submit completed forms to the school office or email them to athleticdirector@dominionacademy.org. Physicals are mandatory for participation and must be turned in before athletes can attend practice.

If your student needs a physical for athletic participation, this is a convenient and affordable option.

Step 4: Pay your Athletic Fees
After your application and paperwork have been submitted, you will receive an invoice for each sport through FACTS at the beginning of the current season.
